In Times of Bereavement

The death of a family member can be a hard time. Below are some points to help you during your time of bereavement.

If death occurs at home

  • Telephone the surgery; details will be taken and you will be advised of what action to take.
  • Contact the Funeral Director.
  • Collect the death certificate from the surgery (please telephone first so that it will be ready).

If death occurs at the hospital

  • The Hospital bereavement team will advise you what action you need to take.
  • Contact the Funeral Director to inform them that their services will be needed.

In all cases of death

  • Take the death certificate to the registrar’s office for the area the death occurred. Also take the deceased medical card and birth certificate, if available.
  • Take the green form (Certificate for Burial or Cremation) to the Funeral Director who will take over the organisation of the funeral.
  • Due to Covid-19 the surgery will email the registrar’s office notifying them of the death and the death certificate will be sent by post to the registrar’s office.
  • The green form will be forwarded to the Funeral Director who will support you in arranging the funeral
